Job summary

Habitat Health seeks a New Participant Advocate to welcome and onboard participants into the PACE program. You will build trusted relationships with participants and caregivers, coordinating with the Interdisciplinary Team to ensure a smooth transition into care and ongoing support.

You will educate clients about services, coordinate appointments and transportation, and document interactions in the EMR. Travel within the service area is required as part of the role.

Qualifications

• Experience with older adults or frail elderly populations.
• Ability to coordinate services and support transitions effectively.
• Strong communication and collaboration with participants, caregivers, and teams.

Responsibilities

• Serve as primary contact during onboarding into the PACE program.
• Coordinate onboarding activities and referrals with IDT and cross-functional teams.
• Advocate for participant preferences and cultural needs.
• Educate participants and caregivers about Habitat Health services and resources.
• Document interactions and actions in EMR and systems.
• Maintain proactive communication to address questions and follow‑ups.
