Job summary

Habitat Health is seeking a New Participant Advocate in California to welcome new participants into the PACE program. You will build relationships with participants and caregivers, coordinate onboarding, and liaise with the Interdisciplinary Team to ensure a smooth transition into care.

The role emphasizes participant-centered support, education about services, and proactive communication to enhance the onboarding experience. A valid driver’s license and EMR experience are expected.

Qualifications

• Bachelor's degree or equivalent in a related field.
• 1+ year experience with older adults or participant advocacy.
• Strong communication and collaboration skills.
• Ability to coordinate services and support transitions.
• Proven ability to work across interdisciplinary teams.
• Valid driver's license and reliable transportation.

Responsibilities

• Build trusted relationships with new participants and caregivers.
• Own onboarding journey and primary point of contact during transition into PACE.
• Coordinate services with IDT and cross-functional teams.
• Advocate for participant preferences and timely solutions.
• Provide education about Habitat Health services and resources.
• Schedule appointments and transportation for onboarding activities.
• Maintain documentation in EMR and other systems.
• Identify trends to improve onboarding and collaboration.
