Job summary

Habitat Health seeks a New Participant Advocate to lead welcoming onboarding for participants entering the PACE program. You will build trusted relationships with participants and caregivers, coordinate the onboarding journey, and partner with the IDT and cross-functional teams to ensure a seamless transition into Habitat Health's care services.

Qualifications

• Bachelor's degree or equivalent in Social Work, Human Services, Healthcare Administration, Public Health, or Psychology.
• Minimum of one year experience working with older adults, frail elderly populations, participant advocacy, care coordination, case management, client services, or a similar participant-facing role.
• Ability to build trusted relationships while delivering compassionate, participant-centered support to diverse populations.
• Coordinate services, navigate complex participant needs, and support successful care transitions through collaboration.
• Strong communication and collaboration skills to engage participants, caregivers, and cross-functional teams.

Responsibilities

• Build trusted relationships with new participants and caregivers to understand goals, preferences, and needs.
• Coordinate onboarding journey as primary contact for transition into the PACE program.
• Collaborate with IDT and cross-functional teams to coordinate services and continuity of care.
• Advocate for participant preferences and cultural needs, identify barriers and support timely solutions.
• Provide education and guidance on Habitat Health services, resources, and onboarding expectations.
• Schedule and coordinate participant appointments, transportation, and follow-up activities.
• Maintain proactive communication with participants, caregivers, and care team.
• Document interactions and onboarding activities in EMR and systems.
• Identify participant trends to improve onboarding processes and collaboration.
• Perform other related duties as assigned.
