Habitat Health is seeking a New Participant Advocate to create a welcoming onboarding experience for participants entering our PACE program. You will establish trusted relationships with participants and caregivers, coordinating with the IDT and cross-functional teams to ensure seamless transitions and ongoing support.

You will educate participants about services, coordinate onboarding activities, and advocate for culturally responsive, person-centered care throughout the care journey.

Qualifications

• Minimum of 1 year of experience working with older adults or participant-facing roles.
• Ability to build trusted relationships with diverse populations and provide compassionate support.
• Experience coordinating services and navigating complex participant needs.

Responsabilités

• Build trusted relationships with new participants and caregivers to onboard into the PACE program.
• Coordinate onboarding journey and act as primary contact during initial transition.
• Collaborate with IDT and cross-functional teams to ensure continuity of care and positive participant experience.
• Advocate for participant preferences, culture, and person-centered care; identify and resolve barriers.
• Educate participants and caregivers about Habitat Health services, resources, and program expectations.
• Schedule appointments, transportation, and follow-up services to facilitate smooth transitions.
