Job summary

Habitat Health is expanding its PACE program and seeks a New Participant Advocate to welcome new participants and coordinate onboarding. You will cultivate trusted relationships with participants and caregivers, partnering with the IDT to ensure a seamless transition into Habitat Health services.

Qualifications

• Bachelor's degree or equivalent in a related field and experience with older adults or participant advocacy.
• Experience coordinating services and navigating complex participant needs.
• Strong communication and collaboration skills with diverse populations.
• Ability to manage multiple priorities with attention to detail and timely follow-through.
• Experience using EMRs, MS Office, and related documentation systems.
• Willingness to travel within the service area and meet health requirements including TB screening.

Responsibilities

• Build trusted relationships with new participants and caregivers to understand goals and support needs.
• Coordinate onboarding during transition into the PACE program.
• Partner with IDT, caregivers, and cross-functional teams to ensure continuity of care.
• Advocate for participant preferences and cultural needs in care planning.
• Provide education about Habitat Health services, program expectations, and available resources.
• Coordinate appointments, transportation, and follow-up services for onboarding.
• Maintain timely documentation in EMR and organizational systems.
• Identify trends and opportunities to improve onboarding processes and participant experience.
