Job summary

Habitat Health seeks a New Participant Advocate to create a welcoming onboarding experience for participants entering the PACE program. You will build trusted relationships with participants and caregivers and coordinate with the IDT to ensure smooth transitions and continuity of care.

You will educate participants about Habitat Health services, manage onboarding activities, and document interactions in the EMR.

Qualifications

• Bachelor's degree in Social Work, Human Services, Healthcare Administration, Public Health, Psychology, or related field, or equivalent experience.
• Minimum of one year working with older adults, frail elderly populations, care coordination, or similar participant-facing roles.
• Ability to build trusted relationships and deliver compassionate, participant-centered support.
• Ability to coordinate services and support care transitions through collaboration.
• Strong communication and collaboration skills with participants, caregivers, and cross-functional teams.
• Strong organizational skills to manage multiple priorities and follow-through.
• Ability to work independently while collaborating with interdisciplinary teams.
• Experience with EMRs and organizational documentation systems.
• Sound judgment, problem-solving, and discretion in sensitive participant situations.
• Valid driver's license and travel ability within service area.
• Meets health requirements (TB, immunizations).
• Aligns with Habitat Health values and operates with integrity.

Responsibilities

• Build trusted relationships with new participants and caregivers to understand goals and needs.
• Coordinate onboarding journey as primary contact during transition into the PACE program.
• Partner with IDT and cross-functional teams to ensure continuity of care and positive experience.
• Advocate for participant preferences and cultural needs with the care team.
• Provide education on Habitat Health services, resources, and program expectations.
• Coordinate appointments, transportation, and onboarding activities.
• Maintain proactive communication with participants, caregivers, and care team.
• Document interactions and onboarding actions accurately in EMR and systems.
• Identify trends and opportunities to improve onboarding processes.
• Perform other related duties as assigned.
