Job summary

Habitat Health seeks a New Participant Advocate to welcome and onboard participants into the PACE program. You will build trusted relationships with participants and caregivers, coordinate onboarding, and collaborate with the IDT to ensure a seamless transition into care and services.

You will educate participants about available resources, coordinate transportation, and document actions in EMR. A patient-centered approach and strong communication are essential for success.

Qualifications

• Experience working with older adults or frail populations in a participant-facing role.
• Demonstrated ability to build trusted relationships and coordinate care for diverse populations.
• Strong communication and collaboration skills for engaging participants, caregivers, and cross-functional teams.

Responsibilities

• Build trusted relationships with new participants and caregivers to understand goals, preferences, and support needs.
• Coordinate onboarding from initial transition to introduction to Habitat Health's services.
• Partner with IDT, caregivers, Referral Coordinator, and teams to ensure continuity of care.
• Advocate for participant preferences, cultural needs, and person-centered care.
• Educate participants and caregivers about Habitat Health services, resources, and support.
• Schedule appointments, transportation, and follow-up activities for a smooth onboarding.
• Maintain proactive communication to address questions and resolve concerns.
• Document interactions and onboarding actions in EMR and other systems.
