Job summary

Humana Inc. is seeking a Telephonic Nurse Care Manager to support patients transitioning into and out of our services. This remote, part-time role (20 hours/week) requires Saturdays and Sundays 10:00 a.m.–8:30 p.m.

EST, with training on weekdays evenings and weekend days. You'll assess clinical needs, coordinate post-acute care, and educate patients about Home Health, Hospice, and Palliative Care. Requires an RN license, Associate degree (BSN preferred), 3+ years post-acute nursing, and strong

Qualifications

• Associates Degree required. BSN preferred.
• RN license (compact state) required; additional state licensures may be needed.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Analytical and able to problem-solve.
• Excellent verbal and interpersonal skills.
• Phone-based communication with empathy.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients recently discharged or needing post-acute services.
• Navigate healthcare options and post-acute offerings, Medicare coverage, billing issues, and resources.
• Make clinical level of care determinations based on discussion, records, and data.
• Follow up and track referral and admission outcomes to maximize admissions.
