Job summary

Haven Home Care is expanding its Eastern Ohio presence and seeking an LPN to supervise Home Health Aides in Ashtabula, Columbiana, Mahoning, and Trumbull counties. This is a part-time, expansion role with pay per visit and mileage reimbursement.

You will document visits, conduct supervisory checks every 60 days, and perform pre-employment testing for aides. The role requires an Ohio LPN license, CPR, reliable transportation, and residency in or near the service area.

Qualifications

• Must be 18+ years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license and auto insurance for work.
• Current CPR certification.
• Background check and testing completed.

Responsibilities

• Prepare accurate documentation according to agency standards.
• Conduct supervisory visits every 60 days to Home Health Aides.
• Complete open visits for new clients.
• Administer pre-employment testing for aide candidates.
