Job summary

Haven Home Care is expanding its services into Eastern Ohio and seeks a Supervisory LPN to lead the onboarding of new clients across Ashtabula, Columbiana, Mahoning, and Trumbull counties. The role focuses on supervisory visits, documentation, and pre-employment testing for aides, with miles reimbursed for travel and a dedicated iPad data plan provided.

Ideal candidates are experienced LPNs, capable of independent scheduling, and comfortable building the client base from zero.

Qualifications

• Must be at least 18 years old
• Graduation from an accredited School of Practical Nursing
• Current Ohio LPN license in good standing
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work
• Current CPR certification
• Successful completion of criminal background check, skills test, and written nursing test
• Residence in Trumbull or Mahoning County preferred

Responsibilities

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates
