Haven Home Care in Eastern Ohio is expanding its trusted care model into Ashtabula, Columbiana, Mahoning and Trumbull counties. We are seeking an LPN to establish presence and grow with our team as we launch services in a brand-new territory.

The Supervisory LPN role focuses on open visits, documentation, and periodic supervision of Home Health Aides every 60 days, with no required skilled nursing or medical assessments. Flexibility and entrepreneurial mindset are essential.

Qualifications

• Graduation from an accredited School of Practical Nursing.
• Experience with scheduling, documentation, and compliance regulations is a plus.

Responsibilities

• Prepare accurate documentation according to agency standards and regulatory requirements.
• Conduct regular supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients.
• Conduct skills tests and written tests for Home Health Aide candidates.
