Job summary

21st Century Home Health Services is seeking a Home Health Speech Therapist/Language Pathologist to serve Vallejo and surrounding areas in Solano County. You’ll collaborate with the Case Manager to provide skilled services for breathing, speaking and swallowing, and to evaluate and treat speech, language, cognitive and fluency disorders.

Qualifications

• CA Speech Therapist License required and unrestricted.
• BLS/CPR certification required.
• Valid CA Driver's License required.
• Experience with home health care preferred.
• Ability to work with an interdisciplinary care team.

Home Health Speech Therapist/Language Pathologist

Territory: Solano County: Vallejo, American Canyon, Hercules and minimal visits to Napa County

What you’ll do:

• In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
• Serve as an educator, patient advocate and member of the interdisciplinary care team.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
