$110,000.00/yr - $125,000.00/yr
Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401(k) (up to 4% match).

All opportunities at 21HHS require field visits to patients in their homes. Remote/work‑from‑home positions are not available unless specifically noted.
Territory: Contra Costa County: Walnut Creek Area.
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate, and member of the interdisciplinary care team.
