Job summary

21HHS is seeking a Home Health Speech Therapist/Language Pathologist (ST) to serve Monterey County — Salinas. You will visit patients in their homes, evaluate and treat speech, language, cognitive and swallowing disorders, and work closely with a Case Manager in an interdisciplinary care team.

Requirements include a CA Speech Therapist license, BLS/CPR certification, and a CA Driver's License. The role offers flexible schedules and autonomous practice within a supportive benefits package.

Qualifications

• Current unrestricted CA Speech Therapist License is required.
• CPR/BLS certification is required.
• Valid CA Driver's License.
• Knowledge of development and human behavior in health care contexts.
• Knowledge of homecare regulations preferred.
• Home health care experience is highly preferred.
• Experience with high-acuity patients is a plus.
• Experience with interdisciplinary teams is highly desirable.
• Case Management fundamentals are highly desirable.
• Self-directed, empathetic and critical thinker with strong interpersonal skills.

Responsibilities

• Provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive and fluency disorders affecting communication.
• Serve as educator, patient advocate and member of the interdisciplinary care team.
• Visit patients in their homes in the field as part of the care continuum.
• Collaborate with Case Manager to coordinate care and services.

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

Please note: All opportunities at 21HHS require being in the field visiting patients in their homes.
