Job summary

21HHS is seeking a Home Health Speech Therapist/Language Pathologies (ST) to provide skilled services to patients in their homes in Solano County, Petaluma. You will evaluate and treat speech, language, cognitive, and swallowing disorders within an interdisciplinary care team.

In partnership with the Case Manager you’ll educate patients and families, advocate for their needs, and help them maintain communication independence while navigating home health regulations.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of development and culture in health care.
• Home health care experience preferred.
• Experience with high acuity patients is a plus.
• Interdisciplinary team experience highly desired.
• Case Management fundamentals highly desired.

Responsibilities

• Provide skilled speech, language, cognitive, and swallowing therapy to patients at home.
• Evaluate and treat communication disorders affecting speaking and understanding.
• Educate patients and families; advocate for patient needs.
• Collaborate with Case Manager and interdisciplinary care team in home health settings.

Job Overview

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.

Please note: All opportunities at 21HHS require being in the field visiting patients in their homes.

Qualifications
