Job summary

21st Century Home Health Services seeks a Speech Therapist/Language Pathologist to deliver in-home evaluations and treatment for speech, language, cognitive and swallowing disorders. You will collaborate with the interdisciplinary care team and act as an educator and patient advocate in the Gilroy/Morgan Hill area.

Field visits to patients’ homes are required, with a focus on achieving functional communication goals while following home health care guidelines and ensuring compassionate,

Qualifications

• CA Speech Therapist License is required.
• BLS/CPR certification is required.
• Valid CA Driver's License is required.
• Knowledge of development, culture in healthcare, and patient care.
• Home health care guidelines preferred.
• Experience with interdisciplinary teams preferred.
• Case management fundamentals desired.

Home Health Speech Therapist/Language Pathologies (ST)

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and member of the interdisciplinary care team.

Territory: Gilroy/Morgan Hill Area

Note: All opportunities require field visits into patients’ homes.

Qualifications

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification

• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team highly desired.
