Job summary

21st Century Home Health Services is seeking a part‑time Speech-Language Pathologist to deliver home health services in Richmond, CA. The clinician provides assessment and treatment for swallowing, speech, language, and cognitive disorders, while collaborating with case managers and an interdisciplinary team.

You will educate patients and families, advocate for patient needs, and participate in a care plan that supports independence and quality of life in a home health setting.

Qualifications

• Current unrestricted California Speech Therapist License
• Current BLS/CPR Certification
• Valid California Driver's License
• Knowledge of development and behavior as it relates to illness/injury; cultural considerations in health care
• Home health care experience preferred
• Interdisciplinary team experience preferred
• Case management fundamentals preferred

Position: Part Time Home Health Speech Therapist/Language Pathology (ST)

Location: Richmond, CA (El Cerrito, San Pablo, Pinole, Hercules, Crockett). This is a fully in-person home health role; remote or hybrid work not available.

Job Overview: In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.

Qualifications

• Knowledge of state and federal homecare regulatory guidelines preferred
• Experience in home health care is highly preferred

• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
