Job summary

21Hhs is seeking a Home Health Speech Therapist in Palo Alto, CA. This position requires in-field visits to patients' homes, evaluating and treating different speech and language disorders while collaborating with case managers.

The ideal candidate will hold a valid CA Speech Therapist License and have experience in home health care, along with strong interpersonal and leadership skills. Enjoy flexible schedules and a supportive work environment with excellent benefits.

• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Location: Santa Clara County (Palo Alto, Cupertino, Mountain View)

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST)

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
