Job summary

21st Century Home Health Services is seeking a Home Health Speech Therapist to evaluate and treat speech, language, cognitive, and swallowing disorders in patients' homes. You will collaborate with Case Managers and the interdisciplinary team and educate patients and families.

Ideal candidates hold a CA SLPT license, BLS/CPR, and a valid CA driver’s license, with experience in home health. The role offers autonomy with flexible scheduling and comprehensive benefits.

Qualifications

• Must hold a CA Speech-Language Pathologist license and BLS/CPR certification.
• Experience in home health care and with high-acuity patients is preferred.
• Ability to work autonomously within an interdisciplinary team.

What you’ll do as a Home Health Speech Therapist/Language Pathology (ST)

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

Qualifications

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
