21st Century Home Health Services (21HHS) seeks a licensed Speech Therapist to provide in-home care across Solano County. You will work closely with Case Managers to evaluate and treat communication disorders, and educate patients within an interdisciplinary team.
In this role you will visit patients in their homes, uphold compassionate care standards, and contribute to a patient-centered care plan while coordinating with family and caregivers.

Territory: Solano County—Vallejo, American Canyon, Hercules and minimal visits to Napa County*
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
