Job summary

AdventHealth in Orange City, Florida is seeking a Part-time Registered Nurse to participate in discharge planning and post-acute care coordination. You will lead multidisciplinary rounds, educate patients and families, assess readmissions, and organize family care conferences to ensure timely transitions and quality outcomes.

Required RN license with 2+ years of hospital nursing experience; Care Management experience is preferred. Benefits start Day One with strong development opportunities.

Qualifications

• RN license is required.
• 2+ medical/hospital nursing experience is required.
• Care Management/Utilization Management experience is preferred.

Responsibilities

• Actively participates in multidisciplinary rounds to review patient status and plans for discharge.
• Educates patients and families about emotional, social, and financial impacts of illness and facilitates access to needed services.
• Assesses readmissions to understand reasons and to improve care transitions.
• Organizes and facilitates patient and family care conferences with the multidisciplinary team.
• Documents discharge planning evaluations, plans, barriers, and post-acute care arrangements.
