• Educates patients and families on illness impacts and mobilizes resources for post-discharge needs.
• Assesses readmission causes and communicates with payors for post-acute authorization as needed.
• Organizes and facilitates patient and family care conferences with the team.
• Documents discharge plans, barriers to progression, and required post-acute services.
