Job summary

AdventHealth in Florida is seeking a skilled RN to lead discharge planning across inpatient, observation and ED settings. You will review patient progress, coordinate post-acute care and communicate needs to patients, families, and payors.

You will develop timely discharge plans, organize care conferences, and ensure smooth transitions while applying CMS requirements. Associates of Nursing required; BSN preferred; 2+ hospital experience.

Qualifications

• RN with active license required.
• 2+ medical/hospital nursing experience.
• Care management/utilization management experience preferred.
• Knowledge of CMS Conditions of Participation for Discharge Planning.

Responsibilities

• Actively participates in rounds to review patient status and discharge plans.
• Educates patients and families on illness impact and community resources.
• Assesses readmission reasons and coordinates post-acute care.
• Organizes and facilitates discharge planning conferences with the team.
• Documents discharge plans and communicates with payors for post-acute authorization.
• Develops contingency discharge plans and ensures timely transitions of care.
