Qualifications

• RN licensure required.
• Minimum 2+ years of medical/hospital nursing experience.
• Care management/utilization management experience preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families about emotional, social, and financial impacts of illness and mobilizes community resources.
• Documents discharge planning evaluations, progress notes, and barriers to progression of care.
