Humana, Inc. through CenterWell Home Health is seeking an E-Referral Specialist for a remote, part-time schedule. You will be the initial contact point for referrals, collect data, ensure compliance, and track the referral process.

Qualifications

• High school diploma or equivalent required.
• Typing 40+ words per minute.
• Proficiency with Microsoft Office (Word, Excel, Outlook, Teams).
• Fluent English with strong communication and attendance.
• Ability to work part-time 29 hours per week and meet deadlines.

Responsibilities

• Monitor multiple e-referral systems to identify new referrals and manage alerts.
• Monitor email for requests to pull referrals and follow up on communications.
• Scan documentation to determine patient eligibility for services.
• Manage referral status and provide weekly status reports.
• Communicate with branch, sales, and leadership for updates and decisions.
• Daily management of ERST log with referrals and ROC decisions.
• Maintain productivity per standards.
• Comply with regulatory requirements and company policies.
