Job summary

Humana, Inc. is seeking an E-Referral Specialist to support the referral process and ensure continuity of care. You will be the initial contact for clients and referral sources, handling data collection, documentation, and follow-up on referrals.

This remote, part-time role requires strong communication, attention to detail, and proficiency with Microsoft Office and EPIC systems. Travel is minimal, with some on-site screenings as needed.

Qualifications

• High school diploma or equivalent.
• Typing speed of 40 WPM or more.
• Proficiency with Microsoft Office (Word, Excel, Outlook, Teams).
• Ability to read, write and speak fluent English.
• Strong math understanding for data analysis and reporting.
• Good decision-making and time-management skills.
• Ability to work with multiple screens/systems and maintain accuracy.

Responsibilities

• Monitor multiple e-referral systems for new referrals and alerts.
• Check email requests to pull referrals from all systems and follow up.
• Scan documents to determine patient eligibility for services.
• Track referral status and provide weekly status reports.
• Communicate with branch, sales, and leadership for updates.
• Maintain ERST log and ROC decisions daily.
• Ensure compliance with policies and regulatory requirements.
