Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver wound care services to residents in long-term care facilities. You will diagnose and treat wounds, collaborate with facility staff and PCPs, and ensure treatment plans align with WV law and best practices.

Qualifications include an active WV NP license and board certification, with wound care experience in geriatric populations. Familiarity with EMR systems and wound VAC devices is preferred, and certification in

Qualifications

  • Current, unrestricted NP license in West Virginia.
  • Board Certification as NP is required.
  • DEA licensure is optional.
  • Clinical background in wound care and geriatric populations.
  • Experience with chronic, infected, or stalled wounds.
  • Ability to diagnose and treat diabetic, arterial, venous, and mixed wounds.
  • Familiar with NPIAP staging for pressure injuries.
  • Knowledge of collagen, alginate, foam and silver dressings; DME knowledge.
  • Experience with wound VAC and other DME devices.
  • Willingness to obtain wound certification if not already certified.
  • Experience in long-term care settings.
  • Proficient with EMR systems.
  • Ability to coordinate patient care within an interdisciplinary team.

Responsibilities

  • Provide wound care services for long-term care patients in skilled nursing facilities.
  • Prescribe medications including controlled substances per WV law.
  • Order, review, and follow up on treatments and diagnostics.
  • Communicate changes in patient condition with nursing home staff.
  • Coordinate with the primary care physician and care team.
  • Contribute to care planning and interdisciplinary rounds.

While an interdisciplinary care team provides medical care to patients in long-term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES:
  • Provide wound care services for long-term care patients residing in skilled nursing facilities.

  • Prescribe appropriate medications, including controlled substances in accordance with WV Law.

  • Order, review, and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with the primary care physician.

WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
  • Current, unrestricted NP license in the state of West Virginia.

  • Board Certification.

  • DEA licensure (Optional)

  • Clinical background in adult, family, or geriatric specialties.

  • Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds

  • Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.

  • Must be comfortable staging pressure injuries according to the NPIAP

  • Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.

  • Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices

  • Strong preference to having wound certification or be open to getting certified

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.

  • Ability to work within an interdisciplinary care team to coordinate patient care
