Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to provide specialized wound services to residents in long-term care facilities. You will diagnose, treat, and manage wounds, ensuring high-quality patient outcomes.

You will prescribe medications in line with WV law, order and review treatments and diagnostics, and coordinate with nursing staff and the primary care physician to adjust care plans as needed.

Qualifications

• Current, unrestricted NP license in West Virginia is required.
• Board Certification in appropriate specialty.
• Wound care experience and geriatric care background preferred.
• Comfort with diagnosing and treating wounds and ulcers.
• Experience with wound vacs and advanced wound care products is a plus.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per WV law.
• Order, review, and follow up with treatments and diagnostic testing.
• Communicate changes in condition with nursing home staff.
• Collaborate with the primary care physician on care plans.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with WV Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of West Virginia.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care
