Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver wound care services to residents in long-term care facilities. The role requires an active Kentucky NP license, board certification, and optional DEA, with strong wound care expertise and interprofessional collaboration.

Responsibilities include prescribing medications per Kentucky law, ordering and reviewing treatments, and coordinating with nursing staff and primary care physicians to maintain and improve patient

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure (Optional).
• Clinical background in adult, family, or geriatric specialties.
• Clinical background in wound care with chronic/complicated wounds.
• Comfort diagnosing and treating diabetic, arterial, venous, and mixed wounds or Kennedy Ulcers.
• Ability to stage pressure injuries per NPIAP.
• Familiar with wound care products (collagens, alginates, foams, silvers, etc.).
• Experience with wound vacs and other DME devices.
• Wound certification preferred or willingness to obtain.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to coordinate patient care within an interdisciplinary team.

WOUND NP ROLE AND RESPONSIBILITIES

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
