Job summary

CareMed Health Solutions seeks a Wound NP to deliver wound care services in long-term care facilities. You will prescribe medications, order and review treatments, and coordinate with nursing staff and primary care physicians to adjust care plans.

Requirements include a Kentucky NP license, board certification, and wound care experience. Familiarity with EMR, wound products, and wound vacs is preferred; DEA licensure is optional.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification as a Nurse Practitioner.
• DEA licensure (optional).
• Clinical background in adult, family, or geriatric care.
• Wound care experience with chronic or complex wounds.
• Ability to diagnose and treat wounds (diabetic, arterial, venous).
• Ability to stage pressure injuries per NPIAP.
• Familiar with advanced wound care products and wound VACs.
• Experience with long-term care settings and EMR.

Responsibilities

• Provide wound care services for residents in long-term care facilities.

We are currently seeking Wound Care Nurse Practitioners to provide wound services to patients living in long-term care facilities.

Wound NP Role and Responsibilities

• Provide wound care services for long-term care patients residing in skilled nursing facilities.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review, and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with the primary care physician.

Wound NP Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure (Optional)
• Clinical background in adult, family, or geriatric specialties.
• Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds
• Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
• Must be comfortable staging pressure injuries according to the NPIAP
• Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
• Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
• Strong preference to having wound certification or be open to getting certified
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to work within an interdisciplinary care team to coordinate patient care.
