Job summary

CareMed Group is seeking a Wound Care Nurse Practitioner to provide wound services to patients living in long-term care facilities.

Current, unrestricted NP license in Ohio with Board Certification is required; DEA licensure is optional. Experience in wound care, long-term care, and EMR use is preferred, with collaboration in an interdisciplinary team.

Qualifications

  • Current, unrestricted NP license in the state of Ohio.
  • Board Certification.
  • DEA licensure (Optional).
  • Clinical background in adult, family, or geriatric specialties.
  • Clinical background of wound care; including chronic, debilitating, stalled, or infected wounds.
  • Must diagnose and treat diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
  • Must be comfortable staging pressure injuries per the NPIAP.
  • Familiar with wound care products; collagens, alginates, foams, silvers, etc.
  • Experience with wound vacs and DME (pressure-relieving devices).
  • Strong preference for wound certification or willingness to get certified.
  • Experience in long-term care setting.
  • Ability to work with an EMR.
  • Ability to work in an interdisciplinary team.
  • Basic coding knowledge preferred.

Responsibilities

  • Provide wound care and primary services for long-term care patients in skilled nursing facilities.
  • Prescribe medications, including controlled substances per OH law.
  • Order, review, and follow up with treatments and diagnostic testing.
  • Communicate with nursing home staff regularly about condition changes and care plans.
  • Collaborate with the primary care physician.
  • Develop and adjust care plans with the team.

PRIMARY AND WOUND NP ROLE AND RESPONSIBILITIES
  • Provide wound care and primary services for long-term care patients residing in skilled nursing facilities.
  • Prescribe appropriate medications, including controlled substances in accordance with OH Law.
  • Order, review, and/or follow-up with treatments and diagnostic testing.
  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
  • and/or the care plan.
  • Collaborate with the primary care physician.
Primary And Wound Np Qualifications And Education Requirements

  • Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
  • Must be comfortable staging pressure injuries according to the NPIAP
  • Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc.
  • Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices
  • Strong preference to having wound certification or be open to getting certified
  • Experience working in a long-term care setting.
  • Ability to work with an EMR.
  • Ability to work within an interdisciplinary care team to coordinate patient care.
  • Basic coding knowledge preferred, not required.
