Job summary

PARKMEADOWS is seeking a part-time Physical Therapist to evaluate and treat patients within a healthcare setting. You will communicate with families and the health team, maintain documentation, and participate in discharge planning as needed.

The role requires a Bachelor's in Physical Therapy, state licensure, and proficiency with Casamba and Point Click Care; Masters/Doctorate is preferred. Duties include supervising PT assistants and contributing to staff training.

Qualifications

• Bachelor's degree in Physical Therapy; Masters/Doctorate preferred.
• Licensed Physical Therapist in the state; CE requirements maintained.
• Proficient with Casamba and Point Click Care systems; strong documentation.

Responsibilities

• Evaluate patients within 24 hours of referral; document reasons if delayed.
• Develop and obtain approval for treatment plans; treat per plan.
• Supervise PT assistants and aides in direct patient care.
• Train nursing staff and participate in discharge planning.
• Record evaluations, daily notes, and progress; ensure policy compliance.
• Communicate patient progress with supervisor and health team.
• Participate in conferences and in-services; facilitate staff training.
• Order/determine needed durable medical equipment; ensure safety.
• Maintain positive relationships with patients, families, and colleagues.
