Job summary

PACS is seeking a licensed Staff Physical Therapist to evaluate and treat patients in a post-acute setting. The role includes documenting services, communicating with families and physicians, and conducting staff trainings to maintain quality care.

The therapist will supervise PT assistants, participate in patient care conferences, and contribute to discharge planning. A state license and PT degree are required, with familiarity in Casamba and Point Click Care preferred.

Qualifications

• Bachelor’s degree in Physical Therapy; Masters or Doctorate preferred.
• Licensed Physical Therapist in the state.
• Proficient with Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours of referral or 1 business day; document reasons for delay.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per physician plan and document progress.
• Supervise PT assistants and students in direct patient care as required.
• Communicate patient progress and plans with supervisor and health team members.
• Record evaluations, daily notes, weekly progress notes, and discharge planning as required.
