Qualifications

• Bachelor's degree in Physical Therapy; Masters/ Doctorate preferred.
• Licensed as a Physical Therapist in the state; keep licensure in good standing.
• Proficient in Casamba and Point Click Care.
• Ability to read technical procedures and policy manuals; good problem-solving.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans from referring physician.
• Supervise PT assistants and students; document progress and changes.
• Maintain records: evaluations, daily/weekly notes, and discharge planning.
• Instruct families and nursing staff in home programs and caregiver training.
• Participate in care conferences and rehabilitation trainings.
