Job summary

PACS is seeking a licensed Physical Therapist to join the rehabilitation team at La Mesa Healthcare Center. You will evaluate patients within 24 hours of referral, develop treatment plans, and supervise PT assistants.

The role requires a PT license, a Bachelor’s in Physical Therapy (Masters/Doctorate preferred), and proficiency with Casamba and Point Click Care. You will collaborate with nursing staff and participate in care conferences and discharge planning.

Qualifications

• Licensed PT in the state.

Responsibilities

• Evaluate patients within 24 hours of physician referral.
• Develop and obtain approval for treatment plans.
• Supervise PT assistants and students in patient care.
• Document evaluations and progress per policy and state practice acts.
• Instruct families and staff for discharge and caregiver training.
