• Bachelor’s degree in Physical Therapy; state licensure required.
• Masters or Doctorate in PT preferred.
• Ability to read technical procedures and policy manuals.
• Computer skills to input data in Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for a treatment plan from referring physician.
• Treat patients per physician plan and document progress.
• Supervise PT assistants and students; provide guidance to staff.
• Document evaluations, treatment notes, weekly progress notes, and discharge planning.
