Job summary

PACS is seeking a Staff Physical Therapist to evaluate residents, design and implement individualized treatment plans, and collaborate with the care team. The role involves supervising PT assistants, training staff, and ensuring compliant documentation in line with facility and payer requirements.

The position requires a Bachelor's in Physical Therapy with a state license; Master’s or Doctorate in Physical Therapy is preferred, and proficiency with Net Health and PointClickCare is desirable.

Qualifications

• Bachelor's degree in Physical Therapy required; advanced degree preferred (MS/PhD or DPT).
• Licensed to practice physical therapy in the applicable state; CPR may be required.
• Proficient with clinical documentation systems (e.g., Net Health, PointClickCare).

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and support staff in resident care.
• Communicate progress and discharge plans with physicians, families, and care teams.
• Complete compliant documentation per facility and payer requirements.
