Job summary

PARKMEADOWS in is seeking a Staff Physical Therapist to evaluate residents, develop treatment plans, and improve mobility and independence. You will communicate with physicians, families, and the health care team, and supervise PT assistants in direct resident care in accordance with the state practice act.

The role requires a Bachelor's in Physical Therapy (Masters or Doctorate preferred), current state licensure, CE compliance, and proficiency with Net Health and Point Click Care.

Qualifications

• Bachelor's degree in Physical Therapy; masters/doctorate preferred.
• Licensed Physical Therapist in the state; CE requirements.
• Proficient with Net Health and Point Click Care.
• Ability to educate residents and families on therapy goals.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT assistants and coordinate with the care team.
• Communicate progress with physicians, families, and nursing staff.
• Provide staff in-services and ensure accurate documentation.
