Job summary

Greater Good Health in Salem, OR, is seeking a Part-Time Nurse Practitioner to provide care in an outpatient clinic and via in-home visits within a 30-mile radius.

You will assess patients, perform annual wellness exams for senior Medicare patients, support HCC coding and care coordination, complete documentation in EMR within 72 hours, and travel up to five days a week as needed. Schedule is 2–3 days per week, 8am–5pm.

Qualifications

• NP with 2+ years of clinical experience in primary/gerontology care.
• Current NP license and DEA prescriptive authority.
• CPR/BLS certification required.
• Home health experience is a plus.
• Knowledge of Medicare HCC coding and managed care preferred.

Responsibilities

• Assess patients' clinical problems and provide care.
• Perform annual wellness exams for senior Medicare patients.
• HCC coding, verification, and risk adjustment.
• Coordinate continuity of care with the care team and other referrals.
• Travel within the Salem, OR area to provide in-home visits as needed.
• Document charts within 72 hours in EMR.
• Cover for other nurse practitioners as required.
• Maintain HIPAA compliance and patient confidentiality.
