Job summary

Greater Good Health in Salem, OR, invites a Nurse Practitioner to join our outpatient clinic and in-home care teams. You will manage high-risk patients, perform annual wellness exams, and coordinate with the care team to prevent hospitalizations.

This full-time role requires 2+ years NP experience, ANCC/AANP certification, DEA, CPR/BLS, and comfort with EMR systems. Travel within the Salem area up to 30 miles, 5 days a week, 8am-5pm schedule.

Qualifications

• 2+ years of NP clinical experience in primary care, geriatrics, or related fields.
• NP certification from ANCC or AANP.
• DEA license and prescriptive authority post-hire.
• CPR/BLS certification and Medicare/Medicaid familiarity.

Responsibilities

• Manage high-risk patients in outpatient clinic and in-home visits.
• Perform annual wellness exams for senior patients and document medical histories.
• Coordinate care with the primary team and other referrals.
• Assist with discharge planning and care transitions.
• Maintain HIPAA compliance and accurate medical records.
• Travel within the Salem, OR area up to 30 miles, 5 days a week.
