Job summary

Greater Good Health is seeking a part-time Physician Assistant to provide high-quality care in outpatient clinics and in-home settings in the Salem, OR area. You will join a collaborative team of clinicians, supporting patient panels, performing wellness visits, and assisting with care transitions.

The role emphasizes senior-focused care, Medicare outcomes, and holistic management, with 2-3 days per week, 8am-5pm, and travel to patient homes as needed within a 30-mile radius.

Qualifications

• 2+ years of clinical NP experience in primary care, geriatrics, internal or related fields.
• Acute high-risk patient care experience is a plus.
• Home health experience is a plus.
• Knowledge of chronic care management and medications.
• Current DEA licensure / prescriptive authority post-hire.
• Active CPR/BLS certification.

Responsibilities

• Assess patients' clinical problems and provide care and consultation.
• Perform annual wellness exams for senior patients in clinics or homes.
• HCC coding verification and risk adjustment.
• Coordinate continuity of care and treatment under primary providers.
• Perform tests and screenings as required.
• Counsel patients on discharge plans and education.
• Develop effective care plans during transitions after hospital discharge.
• Support patients and families with compassionate care.
• Attend IDT meetings and collaborate with care team members.
• Travel within the Salem, OR area up to 5 days a week for home visits.
• Complete charts within 72 hours using EMR/portals.
