Job summary

Greater Good Health in Salem, OR seeks a Part-Time Nurse Practitioner to manage outpatient and home-based care for senior patients. You will partner with clinics as an embedded care team member to prevent hospitalizations, optimize chronic disease management, and support holistic patient plans.

Responsibilities include wellness exams, care coordination, HIPAA compliance, and collaboration with social workers and case managers. Travel within Salem area 5 days/week is required.

Qualifications

• 2+ years of NP clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Acute care experience with high-risk patients is a plus.
• Home health experience (NP or RN) is a plus.
• Working knowledge of chronic care management and medication options.
• Active CPR/BLS Certification.

Responsibilities

• Assess patients' clinical problems and provide care and consultation.
• Perform annual wellness exams for seniors in clinic or at home; obtain medical history and reconcile medications.
• HCC coding, verification, and risk adjustment.
• Coordinate continuity of care; collaborate with primary provider and referrals.
• Perform tests and screenings; document results.
