Job summary

Greater Good Health in Salem, OR is seeking a Nurse Practitioner to join our outpatient clinic team and provide in-home visits within a 30-mile radius. You will manage patient panels, coordinate with the care team, and focus on preventing unnecessary hospitalizations.

Qualifications

• 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Active Nurse Practitioner certification (ANCC/AANP) and DEA licensure/post-hire prescriptive authority.
• CPR/BLS certification and ability to travel within the Salem, OR area up to 30 miles.
• Experience caring for older adults and working in an interdisciplinary team.

Responsibilities

• Assess patients and provide clinical care and consultation.
• Perform wellness exams for senior patients in clinic or home visits.
• Coordinate care and manage patient plans in collaboration with the care team.
• Conduct tests, screenings, and documentation in EMR within 72 hours.
