Job summary
Greater Good Health in Medford, OR is seeking a Part-Time Nurse Practitioner to provide clinical care for high-risk patients. Qualifications
- 2+ years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
- Acute care experience with high-risk patients is a plus.
- Working knowledge of chronic care management and various medication options.
Responsibilities
- Assess patients’ clinical problems and healthcare needs.
- Perform annual wellness exams for senior Medicare patients.
- Coordinate continuity of care and prevent complications.
