Job summary

Housecall Providers in Oregon seeks a Nurse Practitioner - Primary Care to deliver in-home medical services focused on chronic disease and palliative care. Care is provided in patient residences and via audio/video sessions, with potential emphasis on Regular or Relief Care within a collaborative care team.

The role requires an Oregon-licensed NP, DEA and CPR certification, and a commitment to flexible hours including evenings and weekends. Travel within Marion/Polk counties is expected.

Qualifications

• Master’s NP degree and Oregon licensure required.
• DEA license required.
• CPR certification at hire or within 6 months.
• Minimum 2 years of related clinical experience preferred.
• Driver’s license with acceptable driving record.

Responsibilities

• Provide diagnosis, treatment, and follow-up care for chronic disease and palliative care patients.
• Perform comprehensive health assessments and develop proactive care plans.
• Make referrals to social or medical agencies as needed.
• Coordinate care with the care team to ensure continuity of care.
• Triage for relief care and urgent situations as needed.
• Complete medical records per agency protocols and quality metrics.
• Participate in after-hours call schedule.
