Job summary

Greater Good Health is seeking a part-time Physician Assistant to join our outpatient clinic in Salem, OR, with travel to patients’ homes as needed. The role emphasizes high-risk patient management, care coordination, and collaboration with the care team.

Ideal candidates have 2+ years of clinical experience as a NP, current CPR/BLS, and DEA prescriptive authority post-hire. Flexible 2–3 days per week, travel within the Salem area is required.

Qualifications

• 2+ years of clinical experience as a NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Active CPR/BLS Certification.

Responsibilities

• Assess patients' clinical problems and healthcare needs and provide clinical care and consultation.
• Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence.
• HCC coding, verification, and risk adjustment.
• Coordinate continuity of care, prevention and avoidance of complications under the direction of the patient’s PCP and collaboration with other referral sources.
• Complete charts within 72 hours of visits and document in EMR/portals.
