Qualifications

• 2+ years of clinical experience as an NP in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Acute care experience with high-risk patients is a plus.
• Active CPR/BLS Certification is required.

Responsibilities

• Assess patients’ clinical problems and healthcare needs and provide clinical care and consultation.
• Perform an annual wellness exam for senior Medicare patients at clinic or their residence.
• Support patients and their families, providing compassionate care.

Skills

Clinical experience as NP

Home health experience

Comfortable with coding and technology

Ability to build rapport with elderly

Education

Nurse Practitioner certification from ANCC or AANP
