Job summary

Haven Home Care Inc. is expanding its proven home care model to Eastern Ohio, inviting an LPN to work alongside another job as a per-visit role.

You will complete open visits, supervisory visits every 60 days, and pre-employment testing, with pay per visit and per task rather than a fixed hourly wage. The role involves travel to Ashtabula, Columbiana, Mahoning, and Trumbull counties, with mileage reimbursement and a provided iPad.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license and reliable transportation with work coverage.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.

Responsibilities

• Complete open visits and collect documentation.
• Conduct supervisory visits every 60 days to check on Home Health Aides.
• Complete pre-employment testing for Home Health Aide candidates.
• This position does not involve skilled nursing care or medical assessments.
