Job summary

Haven Home Care Inc. in Ohio is seeking an LPN to join as a supervisory role alongside your primary job. You will complete open and supervisory visits, perform pre-employment testing, and document visits for Home Health Aides across Eastern Ohio.

The position is per-visit, not hourly, with pay per supervisory/open visit, skills test, and office time, plus mileage. Initial client volume is small as we build the territory in Ashtabula, Columbiana, Mahoning, and Trumbull counties.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance that covers driving for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open supervisory visits and collect documentation.
• Conduct supervisory visits every 60 days to check Home Health Aides.
• Complete pre-employment testing for Home Health Aide candidates.
• Document thoroughly according to agency standards and regulatory requirements.
