Job summary

Haven Home Care is seeking an LPN for a per-visit supervisory role in Eastern Ohio. You will complete open visits, perform supervisory checks, and conduct tests for Aide candidates, while building a new client base across Ashtabula, Columbiana, Mahoning, and Trumbull counties.

This part-time position offers flexible scheduling with no guaranteed weekly hours, and pays per visit plus mileage reimbursement. Must have Ohio LPN license and driving ability.

Qualifications

• Must be at least 18 years old.
• Graduation from accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and current auto insurance for work.
• Current CPR certification.
• Successful completion of criminal background check, skills test, and written nursing test.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Documentation: Prepare accurate and thorough documentation according to agency standards and regulatory requirements.
• Supervisory and Open Visits: Conduct regular supervisory visits every 60 days and complete open visits for new clients.
• Pre-Employment Testing: Conduct skills tests and written tests for Home Health Aide candidates.
